Preservé Breast Augmentation

 

Preservé represents the most significant advancement in breast augmentation I have seen in over thirty years.

No driving

Day after surgery – OK to drive

How does Preservé differ from a traditional breast augmentation?

In a traditional breast augmentation, full general anesthesia is usually required.

An incision is made, usually, but not always under the breast.

The breast is lifted off the chest wall, whether above or below the muscle, by using an electrocautery which heats and cuts the tissue so that there is no bleeding. This creates a lot of inflammation.

A large space, called a pocket is created. The implant sits in this pocket. There is nothing to keep the implant stabilized in place.

The implant rests against the lower part of the breast. Time and gravity can result in stretching of the lower breast with resultant loss of upper breast fullness.

Sometimes the implant can migrate downward or towards the armpit, resulting in unnatural contours, particularly when lying down. Occasionally, this needs surgical correction.

Larger, heavier implants are required to fill this space. When the pocket stretches and the breast bottoms out, a larger implant may be required to correct this, creating a vicious cycle.

The inflammation from the surgery can lead to other potential complications needing reoperation such as capsule contracture.

Now let’s look at how a Preservé breast augmentation is performed.

NO need for general anesthesia – surgery is performed under light sedation. If you want to be aware and able to converse you can, if you want to be completely unaware of the procedure, that is possible as well.

Local anesthesia is placed into the breast. This makes the breast numb so that you feel nothing during the procedure. Ten minutes is allowed for the medication to take full effect.

A smaller incision is made in the lower breast crease. That is where the cutting ends!

A 2 cm “Channel dissector” is passed through the breast to create a narrow tunnel above the muscle.

Next, a balloon is inserted into the channel and inflated to make a pocket for the implant.

The implant is inserted using a plastic funnel, just as in a traditional breast augmentation.

There is no electrocautery needed because there is no bleeding nor blood loss.

The wound is closed with a few stitches.

You wake up.

The entire time from beginning to end of the procedure is about 30 minutes.

At the end of the procedure, you should have no discomfort at all.

You go home about 30 minutes later.
