Preservé Breast Augmentation Recovery: Your Questions Answered

Medically reviewed by Mark D. Epstein, MD, FACS. Last updated: August 13, 2026

Dr. Mark Epstein has performed 187 Preservé procedures as of July 31, 2026 and has championed fast, precisely defined recovery since 2004, when he introduced One-Day Recovery Breast Augmentation to Long Island. Preservé makes recovery faster still. These are the questions patients ask most, answered from his actual practice protocols.

How long is recovery after Preservé breast augmentation?

For most of Dr. Epstein’s Preservé patients, the meaningful part of recovery is measured in hours, not weeks. Because the procedure preserves the ligaments of the breast rather than cutting them, and because there is essentially no blood loss, most patients feel well enough to resume normal light activities the same day, including going out to dinner. Full unrestricted exercise typically resumes at about three weeks.

What is same-day recovery, and is it realistic?

Same-day recovery means resuming normal, non-strenuous daily activities on the day of surgery: showering, raising your arms overhead, going out that evening. It is realistic because Preservé creates the implant pocket by gently moving tissue aside instead of cutting it. Read more on the Same-Day Recovery page.

Will I need narcotic pain medication?

No. Dr. Epstein does not prescribe narcotics after Preservé breast augmentation, and he tells patients this before surgery, not after. This has been his standard since 2004 with the One-Day Recovery technique, where patient survey data showed 97 percent of patients needed nothing stronger than ibuprofen. With Preservé, discomfort is lower still. If a surgeon tells you that you will not need narcotics but hands you a prescription just in case, ask why.

When can I go back to work?

Patients with desk or non-physical jobs can typically return the next day. Patients whose work involves heavy lifting or strenuous physical activity should plan on modified duty until cleared, usually at two to three weeks.

When can I exercise after Preservé breast augmentation?

Walking is encouraged immediately, including treadmill walking the day after surgery. Aerobic exercise resumes at about two weeks, and unrestricted training, including heavy lifting, at about three weeks. Start slowly and build back up.

Why is Preservé recovery faster than traditional breast augmentation?

Three reasons. First, no muscle is cut or lifted, so there is no muscle recovery at all. Second, the pocket is created by gently separating natural tissue planes with a balloon rather than cutting, so tissue trauma and inflammation are minimal. Third, blood loss is essentially zero, so the body has far less to repair. The comparison is covered in detail on the Preservé vs Traditional page.

Does the type of anesthesia change my recovery?

It can shorten the first few hours. Preservé can be performed wide awake under local anesthesia, under twilight sedation, or under general anesthesia, always with a board-certified anesthesiologist. Patients who choose local or light sedation skip the grogginess and nausea that can follow general anesthesia and are typically ready to leave sooner. See Wide Awake Preservé.

Will I have drains, bandages, or a special bra?

No drains, no pain pumps, no binders, and no special bras. The only dressing is surgical glue along a small incision in the breast crease, about 2.5 to 3 cm (1 to 1 1/4 inches).

What should I avoid during recovery?

Strenuous exercise for the first two weeks and heavy lifting for about three. Nicotine in any form is prohibited before and after surgery because it impairs healing. Beyond that, normal life resumes remarkably quickly; most restrictions patients expect simply do not apply.

How does Preservé recovery compare to Dr. Epstein’s One-Day Recovery technique?

One-Day Recovery Breast Augmentation, which Dr. Epstein has offered since 2004, set a defined standard: nearly all patients back to normal activities within 24 hours, documented by patient surveys. Preservé improves on it. With no muscle involvement and no cutting of breast ligaments, many Preservé patients describe their recovery in terms of an hour or two rather than a day.

Disclosure: Dr. Epstein is a paid investigator in the Motiva U.S. FDA clinical trial and is compensated by Establishment Labs to train surgeons on the Preservé technique.
